Questions about your visit and bill //
Do I need to bring a referral?
> No, you don't have to. Texas law requires a physical therapy referral from medical professional (MD, DO, NP, etc...) in order to provide physical therapy treatment. If you had physical therapy with current condition within a year in the past, we can simply request a referral to your primary care physician. If you have new symptoms and wish to have physical therapy, we will evaluate your condition to see physical therapy would benefit to you during initial visit, then we will encourage you with physical therapy evaluation statement to see your medical doctor to get a referral.
What do I need to bring at the first visit?
> You will expect to complete our patient registration and consent forms if you had not done already. We encourage you to bring valid ID card such as driver's license and medicare/insurance card to verify your eligibility. Most importantly, have your primary care physician's name and contact/address would accelerate our process to get a referral for your treatment. If a patient is under 18, a legal guardian must be present at the initial visit to sign a consent form.
Do I need to have an insurance?
> No, you don't have to. If you have an insurance plan that will reimburse your physical therapy treatment at our clinic, we will submit the claims. You need to pay your remaining deductible or copay at the time of service rendered. If you have an insurance plan that currently we are not in network, we will collect physical therapy charges from you at the time of service rendered and we will help you to make sure you get reimbursement directly to you. If you don't have insurance plan or your insurance plan does not include physical therapy service, you will also get physical therapy treatment at our discount self-pay rate.

What do I need to wear?
> The beauty of Physical Therapy is a therapist will work out with you based on your rehabilitation exercise program that specially established on your particular condition. In addition to manual therapy or spinal adjustment in static position, you will expect to participate in dynamic exercise program during your treatment session. There is no requirements but, loosed or exercise outfit would recommended.
How long does therapy session take?
> At Statera Physical Therapy, you will expect full focused contact with your physical therapist during your session as we provide one on one treatment. Therapy session that includes Initial evaluation / Re-evaluation would take 1 hour and treatment session would take 45 mins to 1 hour.
Do you provide Home Physical Therapy?
> If you are home bound status and require home physical therapy, we recommend / refer you to home health agency. However, if you need ergonomic home/office evaluation for your particular needs, we will arrange home/office visit.
